Creating bookmarks and hyperlinks in Acrobat Pro 5

I can still use Acrobat Distiller as a printer to create PDF output, and use Acrobat Pro to edit the output. I’ve tried lots of free PDF creation software and haven’t found one that resolves these problems. To insert bookmarks (internal links) in an Acrobat Table of Contents. Open the document in Acrobat Pro.
